Insurance & Payment Policy
TheAdvancedMed accepts a variety of insurance plans and also offers self-pay options for patients without insurance coverage.
Insurance Coverage
We accept many major insurance plans, including:
• Aetna
• Cigna
• UnitedHealthcare
• Humana
• Ambetter
• CareSource
• Kaiser Permanente
• Medicare (where applicable)
Coverage varies depending on your individual plan. Patients are responsible for confirming their insurance benefits.
Patient Responsibility
Even when insurance coverage is accepted, patients remain responsible for:
• copayments
• deductibles
• coinsurance
• non-covered services
These amounts are determined by your insurance provider and must be paid according to their policy.
Self-Pay Fees
For patients without insurance or for services not covered by insurance:
Initial Consultation: $159
Follow-Up Visits: $99
Payment is due at the time of service unless other arrangements have been made.

TheAdvancedMed accepts the following forms of payment:

Cash payments may be accepted depending on office policy.
Insurance Claims
As a courtesy, we may submit claims to your insurance company on your behalf. However, insurance coverage cannot be guaranteed.
If a claim is denied by your insurance provider, the patient is responsible for the remaining balance.
Outstanding Balances
Balances must be resolved before scheduling additional appointments. Failure to address outstanding balances may result in appointment delays or restrictions.
Prescription Requests
Medication refill requests may require a follow-up appointment. Controlled medications require additional monitoring in accordance with federal and state regulations.
Policy Updates
This policy may be updated periodically to reflect changes in insurance requirements, medical regulations, or practice procedures.
